My Aged Care is the government entry point for aged care services such as transport and meals. They can do an Aged Care Assessment and will give you an Aged Care number, which you will need to pass on to us.

If you have not yet been assessed, our staff can assist you to arrange this. Having the Aged care number ensures you will receive aged care services at the subsidised rates.
